Directions to Office
I am located in northwest Gainesville on 41st Street, near the Fresh Market, close to Sun Country Gymnastics and Thornebrook Shopping Center. From 43rd Street, turn onto 28th Lane (there is a Florida State Credit Union on the corner), continue down 28th Lane to 41st Street. Look for the sign that says “2830 Thornebrook 3.” I am in Suite J, a green building on the north side. There is plenty of parking.
Arriving for your first appointment
Before we meet for the first time, please fill out all of the paperwork (the packet will be emailed to you from my assistant. It takes approximately 40-50 minutes to fill out, so please plan accordingly!). Bring your completed paperwork, insurance card (if using insurance), and form of payment with you. Please have a seat in the waiting room. I will find you at our appointed time.
